Job Summary
The Client Service Supervisor develops and monitors care plans to ensure clients receive quality care. Acts as the primary liaison between clients, homecare aides, managers, referral sources, and others involved in the client's care.
Essential Duties and Responsibilities
1. Maintain client files and related paperwork.
2. Ensure confidentiality of patient/client and agency information, following HIPAA guidelines.
3. Assign homecare aides based on client needs and care plans.
4. Coach caregivers to ensure high-quality care and address performance concerns.
5. Communicate schedules and changes with clients, aides, referral sources, and management.
6. Conduct in-home visits to assess needs and maintain client relationships.
7. Prepare and submit departmental reports.
8. Document client conditions and services accurately.
9. Maintain records of client satisfaction surveys and visits.
10. Handle client complaints and report significant issues; mandatory reporter for critical incidents.
11. Educate and coach on changes to care plans.
12. Maintain positive relationships with clients, aides, and referral sources.
13. Ensure compliance with laws and company policies.
14. Perform other duties as assigned.
Education and Experience
* Must be at least 18 years old.
* High school diploma or GED required; Bachelor's degree preferred.
* Current PPD or Chest x-ray if applicable.
* Experience with Medicaid, Waiver, or Home Healthcare preferred.
* Additional requirements per state/local rules.
