We are creating a more patient-centred, community-based approach that brings compassionate care closer to home in the last year of life or nearing the end of life.We are expanding services in collaboration with the Macmillan Social Investment Fund and local partners, including:A 24/7 Rapid Community Response and Care ServiceA dedicated End of Life Care phone line for public and professionalsStrengthened voluntary sector partnerships and our Hospital@Home virtual serviceWe have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference with a requirement to cover Cornwall and the Isles of Scilly.Interview date: To be confirmedRole and ResponsibilitiesAs an autonomous practitioner the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. Responsibilities include:Holistic nursing assessment and high quality, safe, evidence-based care to patients and carersMaintaining patients in their own homes where possiblePhysical examination and detailed history takingDiagnosis of deterioration in pre-existing conditions and treatment planningRecognising when deterioration indicates a need for advanced care planning and/or end-of-life (dying phase) careYour experience and skills should enable you to identify when a person is deteriorating and when end-of-life planning is appropriate.About the Trust and Local AreaWe’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health and provide specialist support to people with dementia or a learning disability.
