No on-call, no weekends, and no holidays required.Primary ResponsibilitiesConduct in-home assessments including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical examPerform evidence-based practice screenings including point-of-care testing (as appropriate)Identify diagnoses for care management and active medical management to support treatmentFormulate a list of current and past medical conditions using clinical knowledge and assessment findingsCommunicate assessment findings to inform the member's PCP of potential gaps in careIdentify urgent and emergent situations and intervene appropriatelyEducate members on topics such as disease processes, medications, and complianceYou will be rewarded and recognized for your performance in an environment that provides clear direction and development opportunities.Required QualificationsActive NP or PA license or the ability to obtain by start date.
Licensure must be unencumbered, free of open/unresolved disciplinary actions including probation or restrictions against privilege to practiceFor NPs – Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care; or for PAs – Physician Assistant national certification through NCCPAAbility to complete physical requirements, including lifting a 30-pound bag in/out of a car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform physical assessmentsReliable transportation to complete home visit assessmentsPreferred Qualifications1+ years of clinical experience in the highest level of education, clinical settingMay be requested to obtain additional licensure in other geographic areasExperience in gerontology, cardiology, internal medicine, or endocrinologyHome health care or home visit experienceAbility to travel throughout the state up to 5% of the time; eligible for additional compensation incentivesNote: Employees must be in active regular status and remain in role for a minimum of 12 months from hire/rehire/transfer.
If an employee leaves Home and Community, the student loan repayments will cease.
The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of student loan repayments.Salary and Benefits: The salary range for this role is $89,800 to $176,700 annually based on full-time employment.
Pay is based on factors including local labor markets, education, work experience, certifications, etc.
