* Provide care to patients and their families, developing actions according to the social worker's responsibilities;
* Integrate into the interdisciplinary team, contributing to the development and implementation of the Singular Therapeutic Project (PTS);
* Perform social assessments, identifying socioeconomic, family, and cultural aspects;
* Attend and monitor cases with signs or confirmation of social vulnerability;
* Offer guidance and referrals related to social and previdenciary benefits, documentation, and other demands;
* Participate in meetings and interprofessional visits organized by the unit;
* Support and accompany grieving families, providing qualified listening, guidance, and emotional support;
* Monitor hospital transfer, discharge, and care transition processes;
* Mediate conflicts and address spontaneous demands, promoting active listening and resolution.
Requirements:
* Complete higher education in Social Work;
* Active registration with the Regional Social Service Council (CRESS);
* Basic knowledge in IT, especially Excel;
* Knowledge of legislation related to health and social rights;
* Clear, empathetic, and welcoming communication;
* Ease of working in an interdisciplinary team;
* Postgraduate in health-related areas;
* Professional experience in hospital or health services contexts.
