Job Description:
The role of a Home Care Coordinator involves managing caregiver schedules, handling inbound and outbound calls and texts, and ensuring seamless care coordination. This position is part of a 24/7 scheduling coverage team that works closely with an AI-enhanced platform to streamline the scheduling process.
Key responsibilities include:
* Managing caregiver schedules, call-outs, and no-shows to ensure continuous client coverage
* Handling inbound (and some outbound) scheduling calls and texts from caregivers, clients, and agencies
* Using scheduling and electronic medical record (EMR) systems to efficiently manage client information
* Proactively identifying coverage gaps and resolving scheduling conflicts in real time
* Documenting updates accurately in the system to avoid missed calls or service disruptions
* Collaborating with teammates to ensure smooth handoffs across shifts and 24/7 coverage
Required Skills and Qualifications:
To be successful in this role, candidates must have:
* 1–2+ years of experience in U.S. healthcare operations, patient coordination, or provider support roles
* Proven experience speaking with physicians, clinic staff, or healthcare providers
* Familiarity with EMR/scheduling systems, such as WellSky, AxisCare, ClearCare, or similar
* Tech-savviness and the ability to learn new platforms; comfort working with AI-driven tools
* Strong problem-solving skills with the ability to resolve urgent scheduling conflicts
