BAYADA Home Health Care is seeking a full-time Physical Therapist (PT) or Occupational Therapist (OT) to fill the position of Rehabilitation Associate in our Allentown, PA Senior Living office. As the Rehab Associate, you will lead the field staff in providing superior quality client service, clinical excellence, and adherence to policies and procedures. Rehab Associates are also accountable for internal case management, field staff evaluation and development. The successful Physical Therapist or Occupational Therapist should have prior experience in Medicare Home Health. In this growing and dynamic environment, we offer exciting career paths.Rehab Associate will:Direct and coordinate the care provided by a multi-disciplinary care teamLead field staff in delivering exceptional client service and quality clinical careEnsure adherence to policies and proceduresReview clinical documentation, including OASIS and care plan codingMonitor episodic utilizationAct as internal case manager and client advocateParticipate in hiring, interviewing and orientation of field staffParticipate in on-going education, evaluation and development of field staffEnsure accurate and timely case management, including supervision, authorization management and interdisciplinary communicationMonitor Quality AssuranceVisit clients to conduct clinical assessments and supervise staff as necessaryAssist in developing Clinical Associates to become Clinical ManagersProvide clinical insight and focus on clinical competency of your staffCreate a warm, organized and welcoming office grounded in customer serviceThe PT or OT will have:Current license in good standing, Physical Therapist (PT) or Occupational Therapist (OT)Prior supervisory experienceMinimum of two (2) years community health experiencePrior clinical and managerial experience within a Medicare Certified Home Health agencyComprehensive knowledge of OASIS and the Medicare Conditions of Participation for Home HealthStrong clinical assessment and case management skillsDemonstrated success working independently while taking on increased responsibilitiesAmbition to grow and advance beyond current positionExcellent organizational, interpersonal and communication skillsPrior experience with electronic medical record documentation and reporting systemsRequirements:Licensed in the State of PA required.2-3 years of proven experience in home health care preferred2+ years of experience within a Medicare Certified Home Care Agency preferredHome Care Home Base experience strongly preferred.OASIS certification is preferred.In-depth knowledge of Home Health regulationsIn-depth knowledge of OASIS and Medicare Conditions of Participation for home careDemonstrated expertise in episodic case management and high quality clinical service delivery.Understands and connects with The BAYADA Way.Strong interpersonal, communication, and educational skills.Is prepared with a solution based skillset.
