Job Overview
This position involves collaborating with individuals who face behavioral, psychological, or developmental challenges in Lehigh and Northampton Counties.
The ideal candidate will possess strong communication, writing, and time management skills, as well as the ability to establish and maintain effective relationships with consumers, families, and staff from diverse backgrounds.
Key Responsibilities
* Assist consumers in accessing public and private resources/services specified in their service plans.
* Communicate with consumers weekly to determine progress and revise service plans as necessary.
* Develop and enhance consumer networks of social supports in community settings.
* Provide on-call crisis services on a rotational basis.
* Maintain case records and complete documentation as needed.
Required Skills and Qualifications
* Bachelor's degree in a human services field or relevant Associate's degree.
* Valid Pennsylvania Driver's License and clean driving record.
* Ability to pass background clearances and screenings.
