Educates patients and families to cope with emotional or physical issues, coordinates services like crisis intervention, bereavement, advocacy, referrals, prevention, education, and psychosocial assessments.Job DutiesConducts and documents psycho-social assessments interpreting social, environmental, and medical needs.Manages length of stay through effective discharge planning, proactively addressing barriers.Provides counseling and advocacy to assist patients and families with adjustment issues.Considers options such as palliative care and homecare to promote outpatient health.Monitors risk assessments and implements interventions as needed.Resolves patient care issues compassionately.Negotiates care plans with patients, families, and third-party payors.Communicates with healthcare team and insurance to ensure resource utilization and quality care.Minimum QualificationsMaster’s Degree in Social WorkLess than 1 year experience in an MSW internship programAbility to multi-task and prioritizeProficient in management processes and managed care standardsStrong analytical skills to measure healthcare processes and outcomesEffective negotiation and influencing skillsPhysical DemandsLift and carry 25 lbs., frequent sitting/standing, keyboard use, and role-specific activities like kneeling, bending, squatting, and CPR.This description reflects current duties but may be revised as necessary.
