We are seeking a highly skilled Home Care Coordinator to join our dynamic team. As a key member of our 24/7 scheduling coverage, you will manage caregiver schedules, call-outs, and no-shows to ensure continuous client coverage.
You will be responsible for handling inbound and outbound requests, collaborating with caregivers, clients, and agency staff to keep care delivery on track. You will work closely with our AI-enhanced platform, stepping in when human judgment and problem-solving are needed.
Key responsibilities include:
* Managing caregiver schedules, call-outs, and no-shows
* Handling inbound and outbound scheduling calls and texts
* Using scheduling and EMR systems (WellSky, AxisCare, ClearCare, or similar) plus client's proprietary platform
* Proactively identifying coverage gaps and resolving scheduling conflicts in real time
* Documenting updates accurately in the system to avoid missed calls or service disruptions
* Collaborating with teammates to ensure smooth handoffs across shifts and 24/7 coverage
To succeed in this role, you must have 1–2+ years of experience in U.S. healthcare operations, patient coordination, or provider support roles. You should be a strong communicator, able to explain complex workflows to medical professionals. Additionally, you should be tech-savvy and quick to learn new platforms; comfortable working with AI-driven tools.
